Facility Use Request Form

Complete this form and return it to the church office to request use of the facility.
Electronic Fund Transfer Form
File Size: 24.11 kb
Print, complete, sign and return this form to the church office to have your monthly pledge payment automatically deducted from your checking account. A separate form is needed for each pledge: General Fund, Capital Campaign Phase II or the Child Care Center Endowment. Questions call Beth, 223-6091.
